Error Message: File Page

I've answered all the questions in Step #2 including made sure I entered my bank information correctly. However, I keep getting a message to Revisit Step #2. I don't know what the problem is. I've double checked everything on this page three times. Why is Step 2 keep telling me to Revisit?
