Quarterly tax payment

I was a W-2 employee until June then i became Self-Employed and i filed a 1040 last year, When i logged in to the EFTPS website to pay my Quarterly taxes it asked me to select a tax form and gave me so many choices.

Which one should i choose?

The IRS just changed their website.  You are still paying towards a 1040 return.  Pick 2017 Estimate.
