I withdrew money from an IRA and also converted some money in the same IRA to a Roth IRA. I paid taxes on both when this took place in 2025. I also received the 1099-R for these transactions. Am I required to submit form 8606 when I do my taxes? If yes, will Turbo Tax Premier catch this and fill out/submit the form for me when I do my taxes like it does all other submissions?

Yes, you are required to file form 8606 for the conversion of part of your IRA to a Roth IRA. This is to determine the taxable portion of the conversion (in case you have basis, ie nondeductible contributions in your IRA).
TurboTax Premium Online will automatically fill form 8606 and include it in your tax return. It is important that you enter correctly all the details of the conversion in the TurboTax interview which follows the entry of your form 1099-R.
All my IRA contributions were standard deductible contributions, and most were from a converted 401K. Would I still be required to do the 8606? How would I know any cost basis from and deductible contribution from over 20 years ago?
The basis of your Traditional IRA is the sum of nondeductible contributions you made through the years. Your 401(k) would have no basis as all contributions were from pre-tax money. If all your IRA contributions were deductible, then you have no basis in your IRA.
But TurboTax will automatically generate form 8606 for the conversion to a Roth IRA. As you have no basis, the whole amount converted is taxable.

In any year that you do a Roth conversion, Form 8606 Part II is required to be included in your tax return. All Roth conversions are required to be reported on Form 8606.
You are required to enter the Form 1099-R. Whatever tax withholding is shown on the form is credited on your tax return to be applied to your actual tax liability.
