Yes, you are required to file form 8606 for the conversion of part of your IRA to a Roth IRA. This is to determine the taxable portion of the conversion (in case you have basis, ie nondeductible contributions in your IRA).

 

TurboTax Premium Online will automatically fill form 8606 and include it in your tax return. It is important that you enter correctly all the details of the conversion in the TurboTax interview which follows the entry of your form 1099-R.
