If you never had or applied for health insurance through one of the state Marketplace Exchanges then you would not have received a Form 1005-A.

The IRS probably has incorrect information on you concerning health insurance.  Call the phone number on the IRS letter you received to get this problem resolved.  Have your Form 1095-B and 1095-C on hand when you call.
