My wife received disability insurance/paid maternity leave income. However, she hasn't received form 1099-g. She received what looks like a second W2 instead.

Do I report this as 1099 or W2 income?

Yes, this is correct. The leave is reported on a 1099-MISC or W-2 depending on who paid the benefits. If she received a W-2, the benefits were paid through her employer. You would only receive a 1099-G if she received unemployment in addition to her leave benefits. Please enter the information as it appears and on the form that was sent,enter the W-2 in the W-2 section.
