My W2 only shows my employer's contributions to my HSA in box 12. Should it also include my payroll deductions? If so, what should I do?

My employer contributed $1500 to my HSA and I had payroll deductions to go towards my HSA, lets say $2000. However, on my W2 in box 12 it only says "W 1500". Should it also include my $2000? If so, do I need to get a revised W-2? Or if not, where should my $2000 go on my tax forms?

Yes, since your employer collected your payroll deductions and sent them to the HSA custodian along with their own contributions, the employer should have included both amounts (that is, 3,500 in your example) with the code W in box 12.

 

Yes, you can ask your employer for a corrected W2. You should also make sure that your employer reduced your amounts in boxes 1, 3, and 5 by the 3,500, not just the 1,500 that the employer contributed. If the box 1, 3, 5 amounts are incorrect, do insist on a corrected W-2.

 

Otherwise, if only the code W amount is in error, you have a workaround in TurboTax.

 

In the HSA interview, do the following:

1. Be sure to answer Yes to the question "Did [your name] put any money into an HSA in 2024?"

2. Enter any personal contributions on the next screen (i.e., not the missing $2,000).

3. On the next screen, answer Yes to "Did your employer tell you about any other contributions". 

4. Now, three new lines will appear. In the line "Employer and payroll contributions not reported in Box 12 of your W-2", enter the missing 2,000.
