So my wife won a $1350 slot payout….they gave her a w2-g but we can’t seem to find it….i know the date, amount, casino name, that it was a slot machine, and what 2 forms of ID she provided, just not some of the other W2-g stuff like transaction number or cashier etc…..we have contacted casino but nothing so far….worst case how should I enter all this in turbo tax? Just leave what I don’t know blank or say I never received one etc….thanks

When reporting gambling income, the most important information on the Form W-2G is:
if you can't e-file because of the missing info (TurboTax will probably prevent e-filing if critical info is missing)
Don't lie and say you never got it to an IRS agent (you can lie to the Turbotax program) because that will cause issues:
The IRS has almost certainly received a copy of your W-2G from the gambling establishment.
Dince contacting the Casino hasn't worked you can also contact the IRS and ask for a copy. You can always file without the form, being sure, of course, to accurately report your winnings. Sometimes, you'll get a letter from the IRS asking you to report the amount. Remember that the IRS has the copy in its system as the gambling establishment sent them the W-2G. When this happens, it's not a problem. Simply write back and explain that although you've lost the form, you did correctly report the winnings on your return.
