Do I make a copy of my W2 form when mailing my federal and state taxes?

You should have got more than 1 copy.  But if you didn't then yes, make a copy to mail in with each return.  Make sure you keep one for yourself. And be sure to mail federal and state in separate envelopes.  They go to different places.
