There are three ways I can answer this. The first is just a simple call to your human resources or payroll department and request to get a copy of what you had previously filled out. The second could possibly be as simple as looking at the deductions to make your net pay. In many cases if a national payroll company like paychex is handling your payroll checks then the information you are looking for should be right on the stub. The third way requires you to be a little investigator. You can get the withholding table from the IRS or your state tax authority website. You can backtrack and try to match the deductions from the withholding table & your pay. If you are not familiar with the withholding policy you would not want to use the third solution, however that option is open to you should you need it. Happy hunting.
