Is the federal return only 3 pages?

I need to paper file my federal return because i'm applying for an ITIN number for a relative. After I download the pdf I only see 2 pages of 1040 and 1 additional page of schedule 1. Is this how I should mail it or should i download Federal return with government and TurboTax worksheets? Thanks!

Yes, when you mail in your return, you must attach your W2's.
