It could happen.

Boxes 1, 3, and 5 are Wages for purposes of federal income tax, Social Security tax, and Medicare tax, respectively.

Codes DD and E indicate salary deferrals, that is, they indicate amounts that were taken out of your Wages.

Code DD is the sum of the amount that you and your employer combined to pay for health insurance premiums. It is removed from all three boxes.

Code E is an elective deferral to a 403(b) plan. This is reported in boxes 3 and 5 but not in box 1.

Code BB is are designated Roth contributions under a 403(b) plan. Since it is not tax deductible, it will be in all three boxes.

Note that Social Security is taxed on the first $142,800 in 2021. So whatever is in boxes 1 and 5, Box 3 will not exceed  $142,800.

Box 18 is Wages for a locality - some municipalities have income taxes. Generally, this will be your real Wages, although in states that don't permit Section 125 deductions, it might actually be larger than the original Wages.

So, I don't know your state nor the rest of the information on your W-2, but it is plausible that you could have such numbers on your W-2.
