Publication 15-B describes the rules for employer provided fringe benefits. If your employer provided you with fringe benefits that are not eligible to be tax free, then the value must be included in your W-2 box 1 wages and is subject to income tax and employment tax withholding in boxes 2, 4, and 6.
I have a letter from my employer for Taxable Wellness Rewards. They are not on W-2 where should they be entered on tax form?

If you received compensation that was not included in your W-2 box 1 wages but should have been, you should first ask the employer for a corrected W-2. If they refuse, you can file out form 8919, which will collect the employee half of social security and medicare wages that the employer should have withheld. You would go to the section for "other income" and report the payment, there should be a list of special circumstances and one of them should be "this money was paid by my employer and should have been included in my W-2."
