Yes the 8379 can be filed with the return.

Do I have to file one every year or can I just file separately?  Can I still file an injured spouse form for last year?

As long as there is a debt and you file jointly you will need to file that form if you want any of your refund.

Filing separately is usually much worse for both parties.

And yes, you can still file the 8379 for 2015 .... you will have to mail it in.
