Information from my 1099-DIV, Line 12 (exempt interest dividends), seems to have populated the Arizona Form 140, Line 15 (Non-Arizona municipal interest).

The dollar amount on my 1099-DIV is what shows up on Form 140. I have always had information on Line 12 of my 1099-DIV, but in the past, Line15 on Form 140 has remained blank. TurboTax says the entry on Form 140 “requires a schedule listing the payers and the amount received from each payor.” I am stumped and don’t know how to solve this problem.

When entering your exempt interest dividends, you'll get a few screens asking for additional information.

First, you'll get a screen asking if uncommon situations apply. Check any of these boxes that apply. If you check one of the boxes, you'll be asked to enter the amount that applies.

Next you'll be asked what state your exempt interest dividends are from. Choose the appropriate state here. If your interest dividend didn't come from Arizona, the amount has to be added back to your income on line 15 of your Form 140. Make sure this screen reports the proper state.

Thank you so much for the speedy reply.   Still need some help, please.

I did not manually enter the information for my 1099-DIV.  I had it imported into Turbo Tax from my financial services company.  As a result, I am having trouble finding the screen that asks questions, like “if uncommon situations apply.”

Line 12 on my 1099-DIV only has a dollar amount, nothing else.

thanks again.

Edit the 1099-DIV. Verify the amount is properly being reported in box 12. You may need to check the box that says "My form has info in other boxes".

If the amount is not correct, update the amount and click Continue. If the amount is correct, click continue without making any changes. The next screen should ask for uncommon situations. The screen after that should ask for more info about exempt interest dividends.

Thanks again for your response. I have double checked everything, and still seem to be getting the same “Check this Entry” on TurboTax regarding Arizona Form 140, Line 13.  I will check with my financial advisor.
