I received my 1099-R from my brokerage account with an outdated out of state address. Should I request to correct my address or does it not matter for filing purposes? All other personal information on the form is correct.

As long as your Social Security Number is correct on the form, you do not need to request a corrected form to fix your mailing address. Just be sure that the reporting agency has your correct address moving forward.
The mailing address for the tax form does not affect your return with regard to the tax itself.

Additionally, do we have to prepare a tax return for the that state given that the distribution code on the 1099-R was listed as "G"?

Just to clarify, what do you mean by distribution code G on the 1099R?
Code G on a 1099-R has to do with direct rollovers and nothing to do with the state that issued the 1099-R. See HERE.
As far as preparing a state tax return for the state that issued to you this 1099R, we never prepare state returns for that purpose. Only if you earned income in another state, would you prepare a state tax return reflecting the income you earned in that state. States that issue to you income in any form unless you live in that state never require a state tax return prepared from the state issuing you that income.
