The 540 form. If you're using TurboTax and you go to download the state, it will ask you which state and download what you need.

You should print the entire return and all Schedules to mail with the main tax form (CA Form 540). Attach any tax form showing income tax withheld such as your W-2 form. Below is the mailing address for returns with a refund or no balance due:

My wife is a permanent resident waiting for a SSN. W-7 says not applicable. And we are up against Oct 15th deadline. How should we file jointly if her SSN isn't here yet?
