In box 20 of W-2, should I put the locality name (Ex. Toledo, for Toledo, OH,) or put the SD number associated with the locality?

You should enter whatever is on your W-2. Box 20 is for the locality's name, so 'Toledo' will work fine there, as would the SD name/number.
