When you enter items from Box 12 - it is a 2 part entry.   You enter a code and then an amount.   Select code D (401k plan contributions) and then enter the amount.

The "21" is actually a year designation, which is unnecessary for current year (2021) 401(k) contributions. Simply enter "D" and the amount after "21".

So, I also have this problem and have yet to figure this out. Not sure if it's a mistake yet or if just left it. I am waiting for a call back from the employer. 

Ours says for Box 12: 

DD             $8006.36
D 21          $5.85
D                $1215.85

So is someone able to give me any insight while I wait?

The code D represents elective deferrals under a 401k plan which can be made with pretax or after tax dollars of the employee's salary and is nontaxable. The 21 designation simply refers to the year the  contribution was made and has no bearing on the taxable event for this .Thus, you may go ahead and omit it. I would doublecheck with your employer as to why the two D's are reported on two seperate lines and whether they should be combined.

So would it be okay for me to submit the taxes before speaking to the employer or should I wait?

Please clarify are you doing a 2021 or 2022 tax return? D21 represents 2021

 

In addition to AbrahamT comments, Box 12 amounts with the code DD signify the total cost of what you and your employer paid for your employer-sponsored health coverage plan. Code DD amounts are for informational purposes only—they don't affect the numbers in your tax return.
