I was unable to obtain my W-2 in the mail due to it being delivered to the wrong address. What is your best advice to go about filling out the wages and income portion?

Contact the former employer and get a copy. This is not something you can recreate with pay stubs.

You can also get a copy through the IRS and/or Social Security Administration.
