I received medical leave not family leave. I was curious if I still got a form for that or not because I have heard mixed things.

If you are an employee who received medical leave payments, your employer should report these amounts on an IRS form W-2.  Contact your employer if you have questions about the IRS form W-2 that you received.

If you are referring to another form of medical leave, is this a state run plan?   What state are you referring to?  Please clarify.
