I'm on the payment step: it has 59.99 for TurboTax Premier, plus 39.99 for "California tax return". Is state (CA) e-file included in the 39.99, or extra $$ later?

That would be included in the $39.99 on the payment screen.

The only additional fee that would be charged is if you chose to have your fees deducted from your refund.
