To file a technically complete and correct tax return, you must report all worldwide income received for the tax year.

Knowing that, will there be consequences for not including a Form 1099-K that reports $4.75?  Probably not, but it is possible that you would be contacted by the IRS for not reporting the income.  Either way, be sure to keep the Form 1099-K with your tax records.
