1095-C is not entered in your tax return only a 1095-A. that error code says you had Obamacare/healthcare.gov insurance, you can get the form here. if you did not have such insurance contact it anyway, you may have been the victim of identity theft.

The IRS reviewed the information you provided on your return and determined, based on the information in its database, that you need to include information from Form 1095-A on your return.
You use Form 8962, Premium Tax Credit (PTC) if you received advanced payments toward the cost of a health insurance premium during the year, or to claim a premium tax credit related to health insurance, if you're eligible.
