I cannot find a form D-400V to pay my state tax. I did file the return on line thru you. I now want to pay the taxes due. Do I send the whole return with the payment:?

If you can't find the form, you can actually use the NC website below to create your own personalized Form D-400V.  And, no, you do not send the whole return with the payment - just the form and your check.
