How to enter foreign country name and city in new 2019 tax form for permanent residents?

According to the instructions of 2019 Form 1040, you need to complete the foreign address section only if you have one.  You are not required to enter both US and foreign address.  Generally speaking, tax law is separate from Immigration requirements.  I would suggest you to contact the USCIS for further verification.

The IRS allows you to have a foreign address on your tax return. They usually contacts you through correspondence by mail.  To see how to enter the foreign address in the program, click here: Foreign address

Per IRS, "If you have a foreign address, enter the city name on the appropriate line. Don’t enter any other information on that line, but also complete the spaces below that line. Don’t abbreviate the country name. Follow the country’s practice for entering the postal code and the name of the province, county, or state."

To read instructions, click here: 1040 instructions -  page 14 under Foreign Address.
