Update: This form appears to be required to maintain a dependent care FSA deduction. My understanding is you have to document that you spent at least as much as you used from your FSA. So, thanks Turbotax, for not letting me delete this form!
I delete the form and it just comes right back. I am married filing separately and thus do not qualify for the Child and Dependent Care Tax credit. How do I get this form to go away?

Go to the dependent child care tax credit and tell TurboTax yes that you qualify even though you dont. Then find the child that is in there and delete the entry. Then delete the form.
The form can be deleted at Tax Tools / Tools / Delete a form, but there are one or more entries in the software that immediately re-create the form.
