Help! I never hired a nanny or anything so I never had a care provider for my 9 month old little girl but they require me to put an organization or person into my forms, what do I do

If you did not pay a childcare provider then you can remove that form

Did you have an FSA for childcare at work and an entry in box 10 of the W-2?  If so that is what is triggering the form 2441 and you must address it.
