I took Paid Family Leave (Maternity Leave) in 2016. My company paid for my leave 100% for 12 weeks. I did not get an additional tax form.

If I took Paid Leave  but did not get a special form for it, do I still have to check to box to enter the additional form?

I took Paid Family Leave (Maternity Leave) in 2016. My company paid for my leave 100% for 12 weeks. I did not get an additional tax form.

You should receive some type of tax document from either your company, or perhaps a third-party payer, that reflects this income.  It may be that it just hasn't arrived in the mail yet. I wouldn't file until I had that document, as well as any other documents you're expecting.

Your Human Resources department should be able to advise you on what to expect.

You should receive some type of tax document from either your company, or perhaps a third-party payer, that reflects this income.  It may be that it just hasn't arrived in the mail yet. I wouldn't file until I had that document, as well as any other documents you're expecting.

I just spoke with HR and she said she's never heard of anyone getting an additional form for paid leave. Not sure what to do at this point...

@amandamaccormick Really?  Do you recall if taxes were withheld from your payments?  Was your extra paid leave included in your original W-2 from the company?  I'll run this by some of the other SuperUsers and see if someone has any input.  I'll get back with you.  And congratulations on your new baby!

@Kat thanks! There weren't any "extra" payments. It was just my paycheck, which had the normal amount of taxes taken from it.

Great!  Then you've already covered the income when you enter your W-2 into the TurboTax program.  You should be all set!

Which state are we referring to here? California has special rules about PFL, for instance.

Hello, I live in California and this topic sort of applied to me.  I rec'd 100% pay for my PFL with our newborn son born in 2018. I noticed as I was doing my TurboTax this year that checking "Took Paid Family Leave" increased my CA refund by thousands of dollars.
