Form imported incorrectly. How can I update it?

After importing my 1099-INT form, the recipient's address and ZIP code has 'province' listed as my City, and 'OR' listed as my state, which are both wrong. During the import, it read -
RECIPIENT’S name, Street address (including apt. no.), City or town, state or province, country and ZIP or foreign postal code

And substituted 'province' for my City, and 'OR' for my state. How can I correct this imported form?
