Form 5329 problem in Turbotax software

I need to submit form 5329 to report under withdrawn RMD.  I can complete the form showing the amount underwithdrawn and requesting a waiver.  The form itself does not print in the Required forms for Filing.  The explanation prints but not the form with the numbers.

Have you told the system that you plan to mail in the return?  If you are filing electronically the statement is all that needs to be included.  If you are printing and mailing the system should include the form for you.
