What if I am missing the employer's state ID in Box 15 but they do not operate in my state?
If there is an amount in boxes 15 & 16 but no state ID number simply enter the federal EIN in that box.

That doesn’t help if you’ve never had your ein number so how do we get it if we don’t and never get a w2?
If you do not have a W-2 from a prior year from the same employer and you have no access to the EIN for the employer where you worked in 2021, you will enter any nine digit number (example 11-1111111) so that you can print the tax return. The return will have to be mailed and will not qualify for e-file. You will use Form 4852, which is a substitute W-2 and explain what attempts you have made to get the number. Be sure to include the employer name and address.
