Mailed separately. KY does not accept the 725 LLET form electronically, and it is not sent with the state tax return. That form is specifically for your LLC registration. You will want to print it out, sign it, and send it to the address on the form (and probably include your payment as well, which is also separate from the personal tax return).

I'm in the same situation - KY mailed me a letter for tax year 2016, and after speaking with someone at KY Dept of Revenue, it turns out all subsequent years will be due, so I feel mega hosed by TurboTax right now.
Ugh!!!! I have several years that I need to submit, and now all with penalties. I feel a little hosed as well, but I guess that is my own fault for not knowing what questions to ask.
TurboTax needs to make sure they inform people in the self-employment area that they will need to file these documents separately.
I know how you feel. I barely caught this need to file the 725 separately. And I found that the 2022 TurboTax only printed 8 characters of the FEIN on the 725. Intuit should do a better job.
Does this answer (must file by mail) still hold true in 2024? (or I guess it would be the 2023 tax season)
Looking at Form 725 and just completing Turbo Tax, the questions / answers look the same.
Thanks!
Yes.
The following 725-series forms are available in TurboTax, but must be filed by mail.
These forms are available and may be filed electronically using TurboTax:
