Do I include certain closing costs in the box labeled "Original Cost" or do I only include the sale price I paid to buy the condo in that field?

To calculated the Adjusted Cost Basis of a condo I sold, what costs do I include in the box labeled "Original Cost" since there are separate lines to enter certain closing cost expenses such as title insurance, transfer taxes, document preparation, etc.

That field is for what you paid for the condo originally.  You'll enter all of the costs associated with the sale into the closing cost areas.
