If no Medicare was withheld from your paychecks then those boxes -- such as box 6 -- will be blank on your W-2's. Enter your W-2 exactly as it appears.
You are not asked to enter anything on your Form 1040 about having Medicare.
It depends on the type of Medicare costs you are asking about. Please see can I deduct Medicare premiums for more details.
@wkurtz-kurtz if you are trying to complete form 1099-SSA from your social security payments. if it asks about Medicare premiums, just enter zero.
