This is unclear.   What state taxes or medical expenses are you trying to claim?   What state are you in?   If Social Security was your only income then you are not required to file a federal tax return.  We are not sure why you think you would be able to claim anything on a state tax return.

In general, file your tax return even if you know the tax will be zero.

If you have capital gains OR losses you must file to maintain your deductions.

It depends. Since you have no taxable income in your return, state taxes and medical expenses would not be deductible. Itemized deductions such as state taxes and medical expenses reduce taxable income to reduce the amount of taxes owed in your return.

 

Social Security income by itself is not taxable to you thus you have no taxable income to apply medical expenses or state taxes on the federal and state levels.
