401k rollover filed in wrong state

Hi so I rolled over my 401k to Roth last year. I accidentally used my temporary address for it so now they issued me a  1099-R with my temporary address/State. I called ADP and they said they can't change the address. Will I be in trouble for this?

    No, if you have other information proving that your state of residence is not the address shown on your form 1099-R.

    Should a monthly apartment rent be ok? I travel for work so I’m mostly out of my home state.

    Yes, that would be one element of proof.

     

    However, if you live in more than one home, there are other factors to consider. These factors are:

     

    • The address where you are registered to vote.
    • The address you use on your federal and state tax returns.
    • The address listed on your driver’s license or car registration.
    • The address on file with the U.S Postal Service.
