401K 1099R

Hi! I appreciate the help, I am not sure what to click but I have a 401k through my old employer and I switched companies and now have a new 401k that I moved the funds into. I Am also contributing to my Roth now as well. 
I entered code G for distribution rollover and owe no taxes since I didn’t take any distributions myself but my question is for NJ. 1st question: 

Did you move this money from a 401(k) to a Roth 401(k)?
This includes rollovers from a regular 401(k), 403(b) or 457(b) plan to a designated Roth 401(k), 403(b) or 457(b) plan.
Yes, I moved the money to a Roth 401(k), Roth 403(b), or a 457(b) plan
No, I didn’t (Which I selected)
now the 2nd:

Military pension or survivor's benefits
Disability benefits (under age 65 only)
Three-year rule pension or 401(k) benefits
General rule pension or 401(k) benefits
None of the above (I am thinking none of the above??)

You are correct, you would choose "none of the above".
