3rd party sick pay

I’m trying to get my 2024 taxes filed (I know, it’s late) and the only income I have is 3rd party sick pay from The Hartford. The form I got from them isn’t a regular tax form because there’s no EIN or anything like that. How do I go about filing? This is long term disability paid through my former employer
