It sounds like in spite of living and working in Kentucky, they withheld Indiana taxes. Did you used to live there and not provide an updated address to your employer?
The other two with information appear to be for Louisville where you had withholdings both as a nonresident and a resident. The total Louisville withholdings should be 2.2% of your pay. I'm not sure why they withheld some as a nonresident and some as a resident. That could again have to do with a move.
So you owe $900 to Kentucky. That actually does make sense because you haven't paid any money to Kentucky yet. All your state taxes were withheld by Indiana. The good news is that whatever taxes were withheld in Box 17 of the Indiana W-2 should be refunded to you since you neither lived nor worked in Indiana. The bad news is that to get that refund, you will have to file a nonresident Indiana state return which you otherwise would not have to do. On that return, you would allocate $0 of your income to Indiana. That should result in all the taxes withheld being refunded to you.
I don't know why your employer withheld Indiana taxes, but I would check with your payroll department to make sure they have the most recent address. You may have to fill out a new state form to have them withhold Kentucky taxes.
Thank you for your answer! To clarify, I have never lived in IN, or worked there. Do I still fill out a non-resident state return? Would it basically be my main W-2? Also the other two W-2s (LouMetNR and LouMetR for locality) are blank except for boxes 18, 19 and 20, so unsure of how I should enter them. There's no State/Employer ID. Assumuing the LouMetNR means "non-resident" is that a separate non-resident W-2 as well? If I need to get a corrected W-2 to continue, do I still have to pay 900 today no matter what? Thank you again.
