3 versions of my W-2 from my one employer. Box 15-20 is complete on 1st, (Indiana codes). Other two, 15-17 is blank, locality is LouMetNR on one, LouMetR on other.

I live and work only in KY. How do I enter this? As of now, I owe over 900 in state, and that can't be right. Boxes 18 and 19 in every W-2 mostly different from each other. I'm happy to answer any more clarifying questions. Thanks!
