1099 misc

My wife gets a 1099 misc from work at a barn. On the federal and state it keeps asking for business name, address and naics code. What do I do

If you have and amount in Box 7 of that form you are considered as having a business and will  complete Sch C.  Your business name and address are yours and the code is the business code that comes closest to describing what you do.
