1099-c for a LLC that is closed

I had a business that was an online e-commerce business. It was dissolved about 2 years ago.

I received a 1099-C in the business LLC name this year, do I need to include this on my personal taxes and if so, where?

It is over $600. Credit Card was in the business name.
