What content should be in a Doctor's prescription for exercise equipment, and maintenance of the equipment, where the patient has a documented expense?

This is for situations where there is other documentation of a paid expense. Are there any particular requirements about the content of the Doctor's prescription, where exercise equipment is involved?  The expenses are for (1) the equipment and (2) maintenance expense for the equipment.

What content should be in a Doctor's prescription for exercise equipment, and maintenance of the equipment, where the patient has a documented expense?

There should be some type of letter written by your doctor explaining:

  1. What the patient's medical condition is and the date of diagnosis
  2. Describe medical care and/or treatment without the equipment
  3. The purpose of the equipment and it's maintenance in treating the patient's particular medical condition.

Also if this equipment is installed in your house it may be considered a capital expense. Please see IRS – Pub 502 under section ‘Capital Expense

It would appear you are a tax preparer. If so, understand that TurboTax can not be used by paid tax preparers
