Form 8889 isn’t a form you receive, it’s one that you input.
TurboTax must see a code "W" in box 12 of your Form W-2. If that’s the case, your employer entered this because it believes you have a Health Savings Account and a High Deductible Health Plan.
If you don't have an HSA but have a code “W” on your W-2, you should contact your employer and get a corrected W-2.
If you actually don't have an HSA or HDHP, and there’s no code “W” on Form W-2, open your return in TurboTax and:
Select Pick up where I left off, if that’s on the screen
In the menu on the left, select Tax Tools, then Tools.
In the Tool Center pop-up, select Delete next to Form 8889-T in the list and follow the onscreen instructions.
