Yes, include all of those. You will have 3 different lines to enter with codes C, D, and DD. See this TurboTax FAQ that shows how to enter Box 12 of your W-2.
On the screen asking about my HSA there is one box relating to C. At the top of the page it says, Let's Enter Your HSA Contributions". Afterwards it says what my employer did for the year 2024 and then says, Any Contributions You Personally Made. The box by my employer already has a figure but my box is empty. Right below my employer and above mine it says, Box 12 of your W-2, with a hyperlink. Am I to add from my W-2 to my HSA contribution or that was part of the Employer contribution? In which case, I only add additional?
No, that question about personal contributions is ONLY for amounts that you directly sent to the HSA custodian, NOT through your employer. Once you have entered your W-2, you DO NOT enter the code W amount anywhere else on the return. TurboTax will take care of it.
Code W is the sum of the amounts that your employer made to your HSA added to the amounts you paid by means of payroll deduction.
Next, the code C in box 12 of the W-2 is for the cost of life insurance in excess of $50,000 - your employer can deduct the premium for only the first $50,000, all life insurance above that, you pay for with a deferral.
