pre-tax HSA contributions recorded in box 14 instead of box 12

My employer recorded my HSA payroll deduction contributions in Box 14 and did not record their HSA contributions anywhere on the W2.

 

If they do not change the the W2 - where do I make the entries on my tax form so that the information is correct?

Yes, you should include the total amount that was contributed to the Health Savings Account (HSA) for the year.  All of it is counted toward the contribution limit of the account.

You can manually add the contributions to your HSA if they were not reported in box 12 of your W2. In the Deductions & Credits section of TurboTax, find Medical, and click on Add/Edit or Start next to HSA,MSA Contributions.

 

 

Answer the questions regarding ownership & inherited HSA. You will reach a screen that asks Did you put money into an HSA in 2024? Respond Yes. On the following screen, you will be able to enter the amount of the contributions assuming that the amount for the W2 contributions are zero.

It is okay to put my pre-tax contributions there, even though it says contributions I made personally, "not through your employer" ?

 

Where would I put the employer contributions to my HSA (or is this not important to record on the tax form)?

Keep going to the next screen with the title Did your employer tell you about any other contributions?  

 

Answer 'yes' and three boxes will appear.  

 

Enter the amount reported in box 14 of your W-2 in the one labeled Employer and payroll contributions not reported in box 12 of your W-2.

One last follow-up question....

The employer only recorded my pre-tax contributions in the W2 Box 14.  But, on the Form 5498 I received, the employer contributions were also reported.  

Please confirm - I can put this total (from Form 5498) in the Employer and payroll contributions not reported in Box 12 of your W-2 box.

On form 8889-T, I see the full amount (my contributions and my employers contributions) in the Line 6 Smart Worksheet box labeled Employer contributions.   In the Line 2 Smart Worksheet box, that seems like it should include my contribution amount, it says zero.

 

Double-checking that it is okay to leaves things like this - thank you!

Line 2 of Form 8889 is meant for contributions that you make outside of payroll at work.  This is where contributions that you made by check directly to the HSA would be entered.  In your case it should be zero.
