No Payer's Name listed on 1099-SA

Hello,

I am adding my wife's 1099-SA into Turbo and there is no Payer's name or address information listed in the top left box.  Gross distribution for calendar year is 0.00, as in nothing is listed in box 1.

How do I proceed with this?
