Did you receive a 1099-SA form in the mail from your HSA custodian? If you had made any payments from your HSA in 2019 (like using the debit card), then you should have received this weeks ago.
If you indeed did make payments from the HSA ("distributions") and did not receive the 1099-SA in the mail, then go to your HSA custodian's website and see if you can download the form from there. Otherwise talk to a customer service rep at the custodian.
The information you are looking for (and other vital information) is on that form.

I have my 1099-SA in front of me, but none of the information to respond to that question is on the form. The same information is on the website of the paper in front of me. Any other thoughts?
If you received a 1099-SA for 2019 and there is a value in Box 1 (Gross Distribution), then YES you did use your HSA to pay for something in 2019.
Now, to determine if it is taxable you will continue through the questions to "Did you spend your HSA money on medical expenses only?" The answer to this question will determine the tax consequence.
