Yes I had to fill out both forms I faxed it to them a bunch of times I had to call but I guess if you fax it try not to fax it multiple times. I did that and it delayed it even more because the lady told me they use the more recent one they get.

I do not recommend faxing documents to the IRS unless you have a contact person that will confirm receipt.  As an alternative, I suggest contacting your local IRS office and faxing or dropping off any documents required.
